Responsibilities:
- Conduct house calls to manage patients with acute and chronic medical conditions
- Perform clinical assessments, establish diagnoses, and develop appropriate treatment plans
- Provide medical advice, medication management, and follow-up care
- Deliver care in a professional and safe manner within patients’ homes
- Collaborate with the clinical team to ensure coordinated and holistic patient management
- Maintain accurate and timely medical documentation in compliance with MOH guidelines and clinical standards
Requirements:
- Fully registered with the Singapore Medical Council (SMC) with a valid Practising Certificate
- Experience in primary care, family medicine, acute care, or general practice preferred
- Comfortable working independently in a home-visit setting
- Demonstrates professionalism, empathy, and sound clinical judgement
- Strong communication and interpersonal skills with a patient-centric approach
- Able to commit to scheduled shifts or flexible working arrangements
